Here are links to the files you will need to complete to play. Print them out, Fill them out completely and bring them to practice.  It’s not a problem if you can’t have them ready ahead of time, have a parent come with you to fill out the information at the field.

Physical / Medical History (completed by a Dr. / alternately bring a copy of your physical from school if you already had one this year)
